Use of a galeopericranial flap for the reconstruction of anterior cranial base defects.

چکیده
BACKGROUND To evaluate the efficacy of using a galeopericranial flap for reconstruction of anterior cranial base defects. METHODS In Linkou Chang Gung Memorial Hospital from February 1994 to November 2003, 25 patients who had tumors of the skull base underwent craniofacial resection, and a galeopericranial flap was used to reconstruct the anterior cranial base defect. The galeopericranial flap was developed and based on at least 1 side of the supraorbital or supratrochlear arteries and veins; it was only raised from the scalp after the tumor had been removed. It was then turned inwardly and intracranially and was transposed to lie between the skull base bone and the dura. No free skin or bone grafts were used. RESULTS Two patients experienced flap failure, and salvage was subsequently performed using a free vastus lateralis flap for skull base repair. Thus, the flap failure rate was 8% (2/25). CONCLUSIONS The delicate nature and great pliability of a galeopericranial flap make it unique and competent for reconstructions of anterior skull base defects. Its ready availability, valid strength, and sufficient axial blood supply provide a satisfactory barrier for isolating the cranial cavity from the underlying respiratory tract, with minimal morbidity and mortality.

Reconstruction of small dural defects is independent of which technique is used, and vascularized tissue does not appear to be critical. In contrast, large dural defects have been traditionally repaired using regional vascularized flaps, as they promote healing and diminish complications. Recently, novel regional vascularized flaps that do not require cutaneous incisions have been designed: nas...

Abstract Background: The retroauricular-temporal or Washio flap has been introduced for reconstruction of partial nose and cheek defects, and has many advantages. We decided to evaluate the extended use of this technique in order to repair full thickness nasal defects. Methods: Superficial temporal and retroauricular arteries are identified with Doppler flowmetry. Selection points A, B, C and ...

OBJECTIVES Expansion of the clinical indications for ablative endoscopic endonasal approaches has behooved us to search for new reconstruction alternatives. We present the anatomic foundations of a novel anterior pedicled lateral wall flap (Hadad-Bassagaisteguy 2 or HB2 flap) for the vascularized reconstruction of anterior skull base defects. BACKGROUND Reconstruction of upper extremity deformities continues to be a challenge to the reconstructive surgeon. Various loco regional, distant and free flaps are available for reconstruction. However, each has its own set of advantages and disadvantages. Of the commonly performed local flaps, radial artery forearm flap, and the posterior interosseus artery flap stand out prominently. Rec...

Skull base surgical defects present unique challenges to anatomic and functional reconstruction. Fortunately, many endonasal skull base defects are successfully managed with a variety of local and regional reconstructive techniques. However, when prior surgery or radiotherapy eliminates the use of these local and regional reconstructive options, more elaborate free tissue transfer techniques ar...
